Consent for fiber data construction

I, the undersigned, am the legal owner or authorized agent of the property listed below. I hereby provide consent to STS Broadband and its authorized representatives, contractors, and subcontractors to access and perform construction activities required to install fiber-optic infrastructure on, in, over, or under the property.

Such construction activities may include, but are not limited to:

• Excavation, trenching, and directional boring
• Installation of fiber-optic cables and conduit
• Placement of access panels, vaults, or pedestals

• Temporary storage of construction materials and equipment on-site

I understand and agree to the following:


• The service provider will take reasonable care to minimize disturbance and will restore the property to a reasonable condition after construction.
• This consent does not transfer ownership of any part of the property.

• Any long-term access or utility easements will require separate written agreements.

• This consent remains valid unless revoked in writing by the property owner.
